Medication Safety Post Myocardial Infarction: Transitions Matter

We will review recent literature that details gold standard medication regimens for patients after a Myocardial Infarction. With special emphasis on care transitions, we will discuss various scenarios, looking for areas of potential miscommunication or gaps in care that may preclude patients from being placed on or staying on this Gold Standard treatment.

After participating in this session you should be able to:

  • Understand updates to the gold standard for medications post MI
  • Be able to explain rationale for this gold standard medication regimen.
  • Appreciate the role and importance of medication-focused inpatient discharge counseling and also during the outpatient transition visit.
